How do I change my name?

If you’re changing your name due to marriage, reverting to your former name, or changing to a new name, you’ll need to complete a Change of Name Statement and provide some ID and supporting documents.

Change your name in branch

Visit us in branch and bring the following documents:

  1. Your official photo ID or other ID documents so we can verify your identity. See the full list of ID requirements.
  2. A relevant original change of name certificate OR original certified copies (no older than 3 months) of change of name certificate, such as:
    • Marriage certificate issued by an Australian State or Territory Births, Deaths and Marriages registry or by a foreign government body equivalent
    • Revised birth certificate
    • Change of name certificate
  3. Bankwest Change of Name Statement form. You can print this and complete it, or we’ll print it in branch for you.

Change your name via post

If it’s easier, you can send us your documents via post. You’ll need to mail us the following documents:

  1. The completed Bankwest Change of Name Statement form.
  2. Original certified copies (no older than 3 months) of the relevant change of name certificate, such as:
    • Marriage Certificate
    • Revised birth certificate
    • Change of name certificate
  3. Original certified copies (no older than 3 months) of current identification documents.
  4. Post the documents to: GPO Box E237 Perth WA 6841.
